Families can put their children on a waiting list prior to birth or as soon as they know they will be coming to Fort Leavenworth. Orders are not required to be placed on the waiting list. Waiting lists can be six to eight months. For more information or to talk with someone about care opportunities, call 913-684-5138.
Parent Central uses the information received from the registration packets to determine whether a child needs to attend a Special Needs Resource Accommodation Process (SNAP) meeting which is overseen by the Exceptional Family Member Program (EFMP) and the Army Public Health Nurse. If required, the parent will attend a meeting and share information with an interdisciplinary team to determine appropriate placement and/or accommodations in CYS programs. Your child's registration cannot be completed without the required Medical Action Plan or Diet Statement, please bring current action plans (dated and signed by physician within last year) to your registration appointment to expedite the process. You may download Medical Action Plans by clicking on the forms listed below. Please call Parent Central if you have questions about the SNAP process or Medical Action Plans/Diet Statements, (913) 684-5138. (Note: Only medical action plans and diet statements on CYSS forms will be accepted. Download forms below.) CYS Services does not accommodate special diets based on parent preference. Special Diet Forms indicating Parent Preference will not be accepted. All Special Diets must have a medical or religious reason.
Outreach Services also provides Kids On Site (KOS) for group trainings/functions that take place at approved childcare sites such as Resiliency Center or Frontier Conference Center.
